I have added the * (asterisk) to the username & externalSystemID to indicate the elements that are currently required. I also added a "Legend" text box at the top; please edit the text box as you see fit. I didn't do anything else because it looked like a lot of stray formatting was creeping in and I didn't want to mess anything up.
