Finally got all of my permissions set up to share the invoicing document
that I committed to creating during Friday's call.

This is obviously a first pass...I'm sure many of you will have
comments, corrections, or additions, which are most welcome.  I put some
placeholders in  at the end to capture discussion points that I didn't
feel comfortable expounding on.  And I had some comments in there to
clarify a few things I wrote, but I'm not able to see them anymore.
